to individuals that qualify with
No Monthly Premium.
You may be eligible if you are:
- Enrolled in Medicare Part A
- Enrolled in Medicare Part B
- Enrolled in AHCCCS (Arizona's Medicaid Program)

Hours of Operation:
- 8:00 am to 8:00 pm / 7 days a week from November 15 through March 1
- 8:00 am to 5:00 pm Monday through Friday (except holidays) outside of November 15 through March 1
