Email Outage 9/12/2011 - 9/13/2011
We experienced a problem receiving email to UPH.org email addresses during the time period of 12:00PM on 9/12 until 4:30PM yesterday 9/13. The email gateways that control the delivery of email to UPH.org addresses have been corrected. If you sent an email to a UPH.org addressee during the affected time period we are asking that you please resend, and we apologize for the inconvenience.


Our name has changed:
We would like to share some exciting news and introduce you to your health plan’s new name - The University of Arizona Health Plans - University Care Advantage. The reason for the change is that University Medical Center and University Physicians Healthcare (UPH) merged.

What does this mean to you? It simply means that your health plan name has changed. Your doctor will remain the same and you will get your medical care the same as before. In the future, all of your important plan information will have The University of Arizona Health Plans - University Care Advantage on it. We will still make your health and wellness a priority and will use our new company to provide even better service to you.

We want to make sure you receive the services you and your family need, when you need them. This document is also available in alternate formats or languages. Pre - Enrollment Disclaimers

University Care Advantage (UCA) is a Medicare Special Needs Plan who contracts with the Centers for Medicare and Medicaid Services (CMS) to offer Medicare services and prescription drug coverage. To qualify, you must be eligible for Medicare Part A, enrolled in Medicare Part B & AHCCCS, and live in Pima, Cochise, Pinal, or Santa Cruz Counties. UCA offers Medicare benefits plus additional benefits to help you with dental, hearing, and vision.

You must receive all routine care from plan providers. You must use plan providers except in emergent or urgent care situations or for out-of area renal dialysis. If you obtain routine care from out-of-network providers neither Medicare or University Care Advantage will be responsible for the costs.

You may be able to get extra help to pay for your prescription drug premiums and costs. To see if you qualify for extra help, call:
  • Medicare at 1-800-MEDICARE (1-800-633-4227). TTY/TDD users should call 1-877-486-2048, 24 hours a day/7 days a week.
  • The Social Security Administration at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY/TTD users should call 1-800-325-0778
  • AHCCCS at 1-800-654-5437
